This page has no text yet. It is with our lawyer, and we would rather say so than copy a template that makes claims about your data nobody has checked.
What this document has to state
- The transfer of the diagnosis and the rest of the health data to GoHighLevel as a data processor, naming it.
- The region where the data resides, and whether it leaves the country.
- The processing agreement with the processor, and what it may and may not do with the data.
- That the anonymous rating from the post-treatment survey survives a deletion request: everything identifiable is erased and the star is detached from the person, so that a condition’s average does not change retroactively. If the notice does not state this, that decision does not hold up.
In the meantime, no form on this site sends anything. We do not collect health information without first being able to tell you how it is handled.
