What this document has to state

The real inventory of first-party cookies. The site loads no third-party cookies today and the notice has to say so, rather than copying a template listing analytics that do not exist.
The duration and purpose of each one.
How consent is withdrawn, if there is ever anything to consent to.

In the meantime, no form on this site sends anything. We do not collect health information without first being able to tell you how it is handled.
