Rotator cuff tears without surgery: what can help

Partial tears and tendinopathy, how they differ from a complete tear, and how regenerative treatment can support the tendon.

Rotator cuff tears without surgery: what can help

Short answer

Partial rotator cuff tears and tendinopathy can often be treated without surgery. Physical therapy, activity changes and, in selected cases, regenerative treatment with mesenchymal stem cells can help reduce pain and gain mobility. Research suggests these cells may support tendon healing. An MRI shows whether your tear is partial or complete.

In 30 seconds

  • The rotator cuff is four tendons that keep the shoulder stable and let you lift your arm.
  • Partial tears and tendinopathy are the best candidates for non-surgical treatment.
  • Night pain is often the first thing patients notice easing.

What is the rotator cuff?

Four muscles surround the head of the upper arm bone and join it through tendons. Together they keep the ball centred in the socket and let you lift and rotate your arm. With age, repeated overhead work or sport, these tendons can become inflamed or tear.

Partial or complete tear: why it matters

A partial tear damages part of the tendon thickness; the tendon is still attached. A complete tear goes all the way through. Partial tears and tendinopathy respond best to non-surgical care.

How can regenerative treatment help?

Mesenchymal stem cells release signals that modulate inflammation and may support tendon healing. Research in tendons is more recent than in the knee, and reviews describe a possible protective effect for the tendon when cells are used. In practice, many patients notice less pain at night and more comfortable movement.

Treatment is combined with a physical therapy plan to strengthen the shoulder. At Stemlife, the application takes one day at Hospital Puerta de Hierro Andares.

Who is a good candidate?

  • Partial tears or tendinopathy confirmed on MRI or ultrasound.
  • Shoulder pain for months, often worse at night or when lifting the arm.
  • Physical therapy or injections helped only for a while.

Is this for you?

  • You cannot sleep on the side of the shoulder that hurts.
  • Your MRI or ultrasound shows a partial tear or tendinopathy.
  • You want to avoid or delay shoulder surgery.

Frequently asked questions

Can a partial rotator cuff tear heal?

Many partial tears improve with physical therapy and activity changes; regenerative treatment can support that process in selected cases.

Which study do I need?

An MRI or a shoulder ultrasound that shows the type and size of the tear.

Will I need physical therapy too?

Yes. Strengthening the shoulder is part of the plan and helps you get the most from treatment.

How much does it cost for the shoulder?

The reference range for the shoulder is USD 5,000 to 10,000, confirmed after a free review of your imaging.
